Does Insurance Cover Weight-Loss Surgery? A Guide
Cost is one of the first worries people have about weight-loss surgery — and the good news is that more often than people expect, insurance helps cover it. The process can feel confusing, though, so here's a practical guide to how coverage works, what insurers typically look for, and how we take the legwork off your plate.
The short answer: often, yes
Many insurance plans — including Medicare and Medi-Cal, both of which we accept — cover bariatric surgery when you meet their medical criteria. Coverage varies by plan, so the real question isn't usually "will it ever be covered?" but "what does my specific plan require?" That's a knowable answer, and getting it early removes a lot of stress.
Common coverage criteria
While every insurer is different, most look for some combination of the following:
- A BMI of 40 or higher, or a BMI of 35–39.9 with an obesity-related condition like type 2 diabetes, sleep apnea, high blood pressure, or heart disease (newer guidelines may extend to a BMI of 30–34.9 with type 2 diabetes)
- A physician-supervised diet program (often anywhere from 1 to 6 months)
- A psychological evaluation
- Attendance at an informational seminar
- Documentation of your weight history and prior attempts

What to ask your insurance company
If you call your insurer yourself, a little organization goes a long way. Write down: the name of the person you speak with, the date and time of the call, the reference number for the call, which bariatric procedures are covered under your plan, and the specific criteria you'll need to meet. Having that record can save you real headaches later.
If you're self-pay
